NATIONAL DIABETES PREVENTION PROGRAM

In this CDC-approved lifestyle change program, you will learn to eat healthy, add physical activity into your life, and manage stress. With other group members, you’ll celebrate successes and work to overcome obstacles.

 

Do you have prediabetes?

Approximately 1 in 3 American adults have prediabetes - and often do not know it! Prediabetes may be reversed through weight loss, diet change and increased physical activity - the sooner you know, the sooner you can take action.

Key components of the program include:

  • CDC-approved curriculum with lessons, handouts, and other resources to help you make small steps towards sustainable healthy changes.

  • A trained Lifestyle Coach and National Board Certified Health and Wellness Coach to help you learn new skills, set and meet goals, stay motivated and build confidence to make lasting lifestyle changes - and have fun!

  • Belonging to a small support group of community members (6-12 people) with similar goals and challenges. Together you will learn, laugh, share stories, celebrate successes and work to overcome obstacles - and build relationships that extend long after the group concludes.

The lifestyle change program runs for 1 year.

  • The program meets approximately once a week for the first 4 months. Then twice a month for the next two months. At approximately the 6 month mark, the group meets once a month to help make sure the healthy changes stick.

  • Staying in the program for the full year is essential to help you form new habits. Habits are behaviors that we repeat consistently - but not perfectly. If you make a mistake, your lifestyle coach and group will help you find a way to get back on track.

  • All sessions meet on Zoom.

    Next group sessions begins January 10, 2024

($100 per month for the first 6 months)
